PartnerSHIP for a Healthy Community formally adopted a Strategic Direction Plan in April 2008 with the goal of improving access to quality health care and eliminating health care disparities in Erie County over the next five years. The projects and programs outlined below are reflective of issues of health care concern across our country, our state, and our local area. All of the initiatives are supported by the latest national and regional health care data.

Analyzing the problem

In order to achieve our goal of implementing twenty-six health improvement projects over a five year time span, these initiatives will be executed through the SHIP’s stakeholders and are divided among two task forces: Access to Care and Disparity in Health Education: Health Literacy.

Projects will be implemented in areas with highest poverty- 20% or greater in Erie County, serving diverse populations including those differentiated by race, ethnicity, education, income, disability, geographic location, and sexual orientation.

The initiatives outlined on our Task Force pages are a sampling of what we are working to accomplish in Erie County throughout the next five years. However, we can’t reach our goals of improving access to health care and eliminating health care disparities without the collaboration of caring individuals throughout our community.
